Extensive inferoposterior wall damage which has extended to the right ventricle (can be assessed by the ST elevation in lead V4R). Keep in mind that posterior wall damage is seen as reciprocal ST depression and T elevation in lead V2. The Q wave is not yet visible in posterior wall leads III and aVF, so it is possible to repair the condition with thrombolysis.
