This less than a week-old baby has a fourth-degree systolic murmur in the entire precordium. Echocardiography showed tetralogy of Fallot with a tight stenosis at the infundibulum and the valve. On echocardiography the estimated stenosis gradient was 80-100 mmHg. The pulmonary trunk and branches were sufficiently large for total correction, and at the age of 3 months this child underwent a successful operation. Phonogram shows a diamond-shaped pansystolic murmur suggesting stenosis. Spectrogram contains frequencies in excess of 900 Hz suggesting high flow velocity, i.e. very tight stenosis. Area of auscultation: left third ic (LIC3).
