A distal process in the big toe nail. The primary event is trauma, also causing a subungual haemorrhage, but the nail plate is secondarily invaded by a dermatophyte, T. rubrum. A specific antifungal therapy is indicated only if, in addition to the positive fungal culture, microscopy of the nail plate (KOH) is also positive.
