Trachyonychia, "rough nails", may simulate onychomycosis with distal disintegration of nail plate tissue and some distal onycholysis. The nail surface is "longitudinally sandpapered". Often no background pathology is found, but in "twenty nail dystrophy" there is an association with alopecia areata.
