Full-term healthy baby who was found to have a second-degree systolic murmur in the left third intercostal space at the health check on discharge from the maternity hospital. Echocardiography showed that the oval foramen was open (3 mm) which is a physiological finding at this age. At the age of three days, ductus arteriosus had not yet closed, and pulmonary resistance was decreasing normally. At this stage the pressure difference between the aorta and the pulmonary artery measured by echocardiography was 23 mmHg. Pulmonary resistance remained elevated, which is why there was flow only in systole. The highest frequencies in the spectrogram are at the range of 350 Hz, which matches the flow velocity and pressure gradient measured on echocardiography. This ductus probably closes spontaneously before the age of one year. ECG and four-limb blood pressures were normal. Age 0.0 years, weight 3.0 kg. Area of auscultation: left second ic (LIC2).
