Trichophyton rubrum has invaded and destroyed most of the big toe nail plate in a patient on immunosuppressive medication. Topical therapy is totally insufficient in this case. The immunosuppressive medication allowed only the use of terbinafine as an antifungal. The interaction risks excluded azole drugs.
