Hands are not a common site for dermatophyte infection. Here the patients suffers from Trichophyton rubrum infection of the left thumb. In the figure the distal-lateral onychomycosis is in its initial phase, and is best seen by comparing it with the opposite thumb. See the difference in trimming: the patient has tried to hide the nail problem!
