Onychomycosis of the big toenail is not only a cosmetic problem. Thick hyperkeratosis makes trimming of the nail difficult and in vascular problems (diabetics, elderly people) may be the reason behind pressure ulcers. In a case like this, palliative elimination of the thick nail plate with urea occlusion is recommended, but cure is possible only with effective oral antifungal medication.
